For Patients

Congratulations, you’re going home.
But you’re not going alone.

Your medical team wants your recovery to continue at home. So they’re sending us along to help. We’re Segue, and we provide transitional care for people like you being discharged from in-patient or rehab facilities. Don’t worry, we won’t move in. But you will get a call from your Segue Health Care Coordinator within the first two days you’re home. This is to make sure you have everything you need. At this time, we’ll review your discharge instructions, medicines and follow-up appointments — just to make sure we’re all on the same page. Please tell us about any discomfort and feel free to ask questions. Then we’ll make an appointment for a Segue doctor or nurse practitioner to visit to make sure your recovery is progressing as expected. This usually takes place during the first week.

What We Do

Our in-home transitional care improves recovery and reduces re-admissions.

You’ve been taken good care of by your hospital team. But once you’re discharged, they’re not able to follow you home. That’s where we come in. We’re Segue, and we specialize in caring for patients newly discharged from hospitals, nursing homes and other post-acute facilities.

We stay in touch.

A day after discharge, you will get a call from the full-time Segue Care Coordinator we’ve assigned to the case. He or she will address any problems or immediate needs as well as review medications and other discharge instructions to make sure all is understood and followed.

In-home monitoring.

Then within the first week, we’ll send a nurse practitioner to the home for extensive monitoring that includes:
  • Comprehensive medication reconciliation

  • Safety check of premises

  • Confirmation diagnostic tests have been scheduled and performed

  • Review of test results

  • Answering questions and addressing concerns

  • Educating family or caregiver

We care about you — body, mind, and soul. We understand that physical needs have spiritual, emotional and relational consequences. That’s why our Ministry Team wants to be a resource for addressing issues related to your spiritual health in a loving, Christ-like, and Biblical manner.
